Background: Pregnancy complications are a direct cause of maternal death and the leading cause of maternal death in Indonesia in 2022-2023, the causes of which are complex and diverse, one of which is obesity before pregnancy. Obesity in pregnant women results in ischemic placenta which impacts the pathophysiology of pregnancy complications. Objective: This study aims to determine the effect of obesity before pregnancy on the incidence of complications in pregnancy. Method: The research study used a quantitative analytical approach with a case-control design using secondary data from medical records, purposive sampling techniques, with inclusion and exclusion criteria. Data collection used instruments. Respondents in the case group were 69, the control group 69, a total of 138 respondents. The independent variable was obesity before pregnancy, the dependent variable was complications in pregnancy. Analysis using chi-square test. Results: The results of the chi-square statistical test: p1 56.5% and p2 17.4% p-value = 0.000 or p-value <0.05, OR 6.175, 95% CI: 2.820, 13.519. Conclusion: There is an influence of obesity before pregnancy with the incidence of complications in pregnancy at Prembun Regional Hospital, Kebumen Regency. Women with obesity before pregnancy are 6.175 times more likely to experience complications in pregnancy compared to women who are not obese. The limitations of the research are secondary data sources causing incomplete data requiring longer collection time. Suggestions for using primary data to speed up the collection process. Keywords: obesity, complications in pregnancy, pregnant women.
